    We have list id's (LIDs).  The offset are offsets into the Program
 Segment descriptors described in Program Segment Descriptors (PSD)
 Section PSD. LID 3 is rejected which means that you are not supposed to
 be able to jump to that entry directly but may reach it through program
 control.
